Extreme Wellness Clinic has posted our Patient Medical Form on line. This form is the standard form we ask patients to complete when they visit us for their first office visit. Filling the form out in advance has advantages:
Dr. Pickens can review your medical history prior to your scheduled appointment
More time can be spend with Dr. Pickens while in the office
Patients may fax the form to 801-335-0535 or e-mail us at medicalwonders@keptprivate.com or bring a completed form with them at the time of their appointment; however, it is not required that the form be completed in advance.
